Consent Form for Under 18's

Consent and Participation - I consent to my child’s data being used to contact them/me with regards to the show, understand that participation is voluntary and can be revoked at any point of the programme, with a reasonable notice of 30 days and agree that Magnavision Ltd has the rights to cancel, postpone, relocate the programme or to terminate our agreement in the case of a breach of the agreement or of the code of conduct, and to eliminate the participant from the programme.

Media and Marketing - By agreeing to participate in this show I understand that my child could be asked to provide dance footage, prior to auditions, could be photographed or videoed as part of their performance and participation in the programme and understand that their name and/or age could also be published in marketing materials for the show. I also understand that I (or anyone present under my supervision, such as, other family members, children, friends etc) am not permitted to take photographs or videos of the location/rehearsals/performances, or upload them to any Social Media platforms including but not exclusively WhatsApp, Instagram, Facebook, Youtube, Snapchat etc, without explicit permission from the team at Magnavision Ltd.

Health, Safety and Conduct - All medical conditions/allergies have been declared and all appropriate medication will be carried by the child or their guardian and I understand that in the event of an injury Magnavision Ltd will not be liable for any injury caused on the premises or as any part of the show/rehearsals etc.

Travel and Identification - I understand that we may have to travel to the locations for rehearsals, performances etc and may be asked to provide proof of age/some form of identification, upon request or upon arrival at the set/shoot.

By signing this form I agree that my child, anyone else accompanying us and I, will obey the Code of Conduct and understand that a breach of this Code gives Magnavision Ltd the power to exercise their right to terminate our contract and prevent my child/I from further participating in the show. In the case of a breach, it is at the discretion of the team at Magnavision Ltd, how they wish to deal with violations of the Code of Conduct, whether that be a warning, termination of the agreement or any other disciplinary actions.
